I am a resident of Singapore possessing a valid NRIC or FIN.
I have not and have never been tested positive for COVID-19.
I have not had a persistent cough, fever (including raised temperature), diarrhoea, sore throat or any flu-like symptoms in the last 30 days.
I have not been in contact with any individual suspected or confirmed to have COVID-19 in the last 30 days, unless I am required to be in contact with such patients during the course of my work.
I have not been directed or advised to self-isolate due to COVID-19, or to self-quarantine based on medical advice or on government’s notices or orders, in the last 30 days.
I have not travelled outside of Singapore for the last 30 days.
